Contact Us Online Via Our New Online Service

We are happy to announce that from Tuesday, May 4th you will be able to contact us online for none urgent medical and administration queries including sick notes, repeat prescriptions, to request medical advice and any other none urgent requests.
 
We understand our phonelines are extremely busy and would encourage all patients (if able) to use this new online service.
 
By visiting our website and answering a few short questions someone from the surgery will get back to you or action your request within 3 working days.
 
Please DO NOT use this form for urgent requests.
 
Once submitted, forms will be read during office hours only (Monday – Friday 8.30-18.00).
 
Forms will NOT be read on the weekend or bank holidays. They will also not be read Monday – Friday between 18.00pm-8.30am
Please contact 111 during out of hours of 999 for medical emergencies.
