Any new patients applying to register with our practice can either apply online via https://gp-registration.nhs.uk/C88023 or will be required to complete a new patient questionnaire which should be handed in to a receptionist with two other proofs of identity*, as follows:-
*You will not be refused registration or appointments because you do not have a proof of address or personal identification at hand. Please speak to a member of the team for further details.

We have a team of doctors, nurses and health visitors supported by administrative and reception staff who aim to help you with any current or future health problems and disease prevention.
We accept patients who live in the S20 post code area. To register at the practice you will need to complete an NHS registration form. This can be obtained from reception and one will need to be completed for each member of the family. Patients will also be asked to complete a health questionnaire and an ethnicity form; again a separate form will need to be completed for each member of the family.
It is advisable to register a new born baby as soon as possible. The hospital or midwife will have issued the parents with an NHS number. This information will need to be completed on the registration form.
A patient can register as a temporary resident if they are resident in the practice catchment area for more than 24 hours but not more than 3 months.
There is disabled access via a ramp from the car park to the front door.
